Salam alaikum Answer Wa alaikum assalaam wa rahmatullahi wa barakatuh, The word is actually two words, the first being the two letter word for drawing attention and the second the word . The first medd is then because there is an alif at the end of a word and a hamzah at the beginning of the next word. In the reading of Hafs ‘an ‘Aasim min tareeq Ash-Shatibiyyah this can be lengthened 4 or 5 vowel counts, but in Tareeq At-Tayyibah, it can be lengthened two vowel counts. The second medd is because the medd letter (an alif) is followed by a hamzah in the same word. Those reading the first medd with two vowel counts and the second medd with four vowel counts are reading from one of the ways of Hafs ‘an ‘Aasim from Tareeq At-Tayyibah. Please also see the last part of the following lesson (you will have to scroll down): http://www.abouttajweed.com/al-mudood_part_6_medd_waajib_jaa'iz.htm Wa iyyaakum.
